Description
Arising from the dorsal midbrain, the trochlear nerve (CN IV) wraps around the cerebral peduncle and courses anteriorly in the ambient cistern along the lateral aspect of the brainstem. From an inferior view of the male brain, the cerebral hemispheres sit superiorly, while the cerebellum lies posterior and inferior to the occipital lobes, framing the pons and medulla from which the remaining cranial nerves emerge. Multiple cranial nerve roots are visible as they leave the ventral brainstem, creating a useful contrast with CN IV’s atypical posterior exit and long intracranial trajectory toward the orbit. CN IV matters because its anatomy explains its vulnerability. The nerve’s long subarachnoid course and slender caliber make it prone to injury with closed head trauma, often presenting as a superior oblique palsy with vertical diplopia that worsens on looking down and in, classically noticed when reading or descending stairs. In the cisternal segment it runs near the posterior cerebral and superior cerebellar arteries and the tentorial edge, relationships that are routinely referenced when localizing deficits from tentorial meningioma, aneurysmal compression, or microvascular ischemia, and when interpreting brainstem lesions that involve the trochlear nucleus in the midbrain tegmentum. Use this inferior brain and cranial nerve plate in neuroanatomy teaching blocks, ophthalmology modules on ocular motility and strabismus, or neurology content that differentiates nuclear, fascicular, and peripheral CN IV lesions. It also fits surgical education materials on tentorial and petroclival approaches where preserving the trochlear nerve is a constant concern.
