Description
Arising from the dorsal aspect of the midbrain just inferior to the inferior colliculus, the trochlear nerve (CN IV) is traced as a slender yellow filament that wraps anteriorly around the cerebral peduncle toward the ambient cistern. Its course runs along the free edge of the tentorium cerebelli, staying superior to the cerebellar hemisphere and posterior to the cerebral hemispheres before progressing toward the lateral wall of the cavernous sinus. Nearby cranial nerves emerging from the brainstem are also present for orientation, with the cerebellum inferior and posterior and the brainstem centrally continuous with the upper cervical spinal cord. Small nerve. Long route. Clinical teaching often struggles with CN IV because its anatomy breaks the usual rules: it is the only cranial nerve to exit the brainstem dorsally and the only one to decussate completely, so a trochlear nucleus lesion in the midbrain produces a contralateral superior oblique palsy, while a lesion of the nerve after it exits the brainstem causes an ipsilateral deficit. That distinction matters in neuro-ophthalmology when localizing vertical diplopia and hypertropia, and it matters in operative planning because the nerve’s tentorial relationship places it at risk during approaches that mobilize the tentorium or work in the ambient cistern, including procedures near the tentorial edge and upper petroclival region. Neuroanatomy instructors can drop this plate directly into cranial nerve lab practicals to anchor a discussion of the trochlear nucleus, dorsal midbrain exit zone, and the long intracranial subarachnoid segment.
