Description
Frontal alignment of the female lower limbs is presented from pelvis to feet, with the femoral shafts converging medially toward the knees and the tibial shafts diverging laterally below the joint line, creating the characteristic genu valgum silhouette. Medial femoral condyles approximate while the distal legs angle outward, increasing the apparent Q-angle at the knee and shifting the weight-bearing line laterally relative to the tibial plateau. Patellae sit on the anterior distal femora, and the ankles and forefeet remain in neutral position so the coronal-plane deformity reads clearly from knee to toes. Skin surface contours allow you to infer the underlying quadriceps, patellar tendon region, and tibial crest without obscuring bony landmarks. Coronal malalignment matters because it changes compartment loading and patellofemoral tracking, issues that surface in clinic long before radiographs get ordered. In genu valgum, the lateral compartment of the knee often absorbs higher compressive forces, predisposing to lateral compartment osteoarthritis and exacerbating patellar maltracking with anterior knee pain during stairs or squatting. Orthopaedic assessment commonly quantifies this with an intermalleolar distance on standing exam and with full-length, weight-bearing hip to ankle radiographs to measure the mechanical axis and plan guided growth, osteotomy, or arthroplasty alignment targets.
