Description
Running along the radial (thumb) side of the forearm, the abductor pollicis longus (APL) is shown as a superficialized muscular belly beneath intact skin, tapering distally into a tendon that courses toward the radial styloid and base of the first metacarpal. Proximally, its fibers lie lateral to the ulna and posterior to the radius in origin, but the anterior perspective brings its contour forward in relation to the palpable radius, carpal region, and the thenar aspect of the thumb. The radius remains lateral and the ulna medial in anatomical position, with the elbow superior and the hand and phalanges distal. Skin landmarks matter. Clinically, the APL tendon’s passage across the radial styloid into the first dorsal extensor compartment, typically shared with extensor pollicis brevis, defines the pain generator in de Quervain tenosynovitis, where thickening of the tendon sheath provokes radial wrist pain with thumb abduction and grasp. This anterior presentation helps correlate a patient’s point tenderness just proximal to the radial styloid with the underlying tendon trajectory, and it also frames nearby structures at risk during injections or surgical release, including the superficial branch of the radial nerve and branches of the radial artery around the anatomical snuffbox. A frequent anatomic variant, an accessory APL slip, can complicate decompression and explains persistent symptoms when only one tendon band is released.
