Description
Anterior neck anatomy is centered on the musculus scalenus anterior, running inferiorly from the anterior tubercles of the transverse processes of C3 to C6 to its insertion on the scalene tubercle and superior surface of the first rib. Superficial and slightly lateral, the sternocleidomastoid forms the dominant strap on each side, while the mandible and maxilla frame the superior field and the clavicles mark the inferior border of the composition. Medial to the scalene region lie the cervical vertebral bodies and intervertebral discs, with the upper thoracic inlet and first rib contouring the caudal margin. Clear landmarks. For teaching and procedural planning, this anterior perspective is the one clinicians mentally reconstruct when orienting at the root of the neck: the brachial plexus trunks and the subclavian artery pass posterior to the anterior scalene in the interscalene interval, while the subclavian vein runs anterior to it. That relationship explains why an interscalene brachial plexus block targets the groove between the anterior and middle scalene, and why anterior scalene hypertrophy or fibrous bands can contribute to neurogenic thoracic outlet syndrome without necessarily compressing the vein.
