Description
Positioned on the lateral neck, the musculus scalenus anterior runs inferolaterally from the anterior tubercles of the transverse processes of C3 to C6 to its insertion on the scalene tubercle of the first rib. Medial and slightly posterior to it lie the cervical vertebrae and prevertebral musculature, while the clavicle, first rib, and superior thoracic aperture sit inferiorly as the muscle transitions from cervical spine to rib cage. The sternum and ribs provide an anterior thoracic frame, and the scapula and clavicle anchor the shoulder girdle just lateral and inferior to the scalene complex. Bone landmarks are crisp. Clinically, this lateral perspective is the one most readers need to understand the interscalene interval, where the brachial plexus trunks and the subclavian artery pass between the anterior and middle scalene, while the subclavian vein remains anterior to scalenus anterior. That relationship explains the exam findings and procedural risks in thoracic outlet syndrome, and it is also the anatomy that guides an interscalene brachial plexus block, where needle trajectory stays posterior to the sternocleidomastoid and avoids the phrenic nerve coursing on the anterior surface of scalenus anterior. An accurate first rib and clavicular relationship matters. It determines where compression occurs. Use this asset in gross anatomy and regional anesthesia teaching, in atlas plates covering the root of the neck, or in clinical graphics for thoracic outlet evaluation and brachial plexus block technique.
