Description
Seen from the anterior aspect, the lower lumbar vertebrae and adjacent pelvis frame the lumbar plexus within the posterior abdominal wall, with the femoral nerve forming from posterior divisions of L2 to L4 and descending inferolaterally toward the iliopsoas groove. From the femoral nerve, an articular branch courses distally and medially, tracking toward the anterior hip joint capsule near the anterior inferior iliac spine and the iliofemoral ligament region. The cauda equina and exiting lumbar nerve roots provide proximal context, while the hip bone contours (ilium, pubis, and the acetabular margin) define the joint target. Anterior hip pain often has a neural component, and this small articular branch explains why irritation around the iliopsoas, the femoral triangle, or anterior capsular distension can refer pain to the groin and anterior thigh. This is a useful map for differentiating femoral neuropathy from lumbar radiculopathy: L2 to L4 root involvement can mimic hip joint pathology, while femoral nerve injury may spare the capsule depending on branch pattern.
