Description

Arising from the posterior divisions of the L2 to L4 ventral rami within the lumbar plexus, the femoral nerve is traced as it forms in the substance of psoas major and then emerges along the muscle’s lateral border. From there it descends inferiorly in the groove between psoas major and iliacus, deep to the iliac fascia, and approaches the inguinal ligament just lateral to the femoral artery. Distally in the proximal thigh, the nerve fans into anterior divisions supplying the sartorius and cutaneous territory, while deeper branches track toward the quadriceps femoris and continue as the saphenous nerve medially. This anterior relationship is the one clinicians rely on when navigating the femoral triangle: nerve lateral, artery intermediate, vein medial. Order matters. It clarifies why femoral nerve block and fascia iliaca compartment block target the plane deep to fascia iliaca near the inguinal crease, and it also frames iatrogenic risk during femoral arterial puncture, catheterization, or groin surgery where an overly lateral needle trajectory can injure the nerve. Psoas or iliacus hematoma provides the other classic correlation, compressing the femoral nerve and producing weak knee extension with reduced patellar reflex. Use this plate for teaching lumbar plexus organization and anterior thigh innervation in gross anatomy, regional anesthesia modules, and surgical anatomy of the inguinal region, and for illustrating clinical text on femoral neuropathy, meralgia paresthetica differentiation, or post-procedural groin pain after vascular access.
