Description
Running along the anterior neck, the sternocleidomastoid forms a prominent lateral boundary while the platysma lies superficial to the deeper cervical fascia, framing the carotid triangle where the common carotid artery bifurcates into the internal and external carotid arteries. Fine neural branches in the carotid plexus track on the adventitia of the carotid vessels, with the carotid body nerve (Hering’s nerve) coursing superiorly from the glossopharyngeal nerve toward the carotid body at the posteromedial aspect of the bifurcation, adjacent to the carotid sinus. Inferiorly, the clavicle and superior thoracic inlet anchor the composition, and the trachea sits midline, medial to the vascular sheath. Orientation is anterior, with medial structures clustered around the laryngeal inlet and lateral structures dominated by the sternocleidomastoid. Hering’s nerve matters because it carries chemoreceptor and baroreceptor afferents from the carotid body and carotid sinus, a pathway central to short latency cardiovascular reflexes and ventilatory drive. Irritation of the carotid sinus during neck dissection, carotid endarterectomy, or retraction near the bifurcation can precipitate reflex bradycardia and hypotension, and carotid sinus hypersensitivity remains a concrete cause of syncope in older patients.
