Description
Dominating the anterior thorax, the heart is shown with the superficial coronary venous network coursing in the epicardial fat and converging toward the coronary sinus in the posterior atrioventricular (coronary) sulcus before it opens into the right atrium. Running in parallel with the anterior interventricular sulcus, the great cardiac vein ascends alongside the left anterior descending artery, then turns left in the coronary sulcus toward the left atrioventricular groove. Smaller anterior cardiac veins cross the anterior surface of the right ventricle and drain directly into the right atrium, while venules and tributaries outline the drainage territories of the right and left coronary circulations. Color coding distinguishes veins from arteries, and major inflow and outflow tracts such as the ascending aorta, pulmonary trunk, and superior vena cava frame the cardiac silhouette. Anterior coronary veins matter because they dictate where venous return can be interrupted, accessed, or inadvertently injured during common interventions. Cannulation of the coronary sinus for retrograde cardioplegia, placement of a left ventricular lead for cardiac resynchronization therapy, and electrophysiology procedures that map near the atrioventricular grooves all depend on recognizing how the great cardiac vein and its tributaries track the coronary arteries and sulci. Small but clinically real variants in the size and course of the anterior cardiac veins, and their relationship to the right atrial appendage, can complicate right-sided atriotomy planning.
