Description

Centered in an anterior abdominal view, the ascending colon rises on the patient’s right to the hepatic flexure, the transverse colon crosses superiorly, and the descending colon tracks inferiorly on the left into the sigmoid colon and rectum within the pelvis. Superficial colic venous channels accompany the taeniae coli and marginal arcade, coalescing medially into named tributaries that form the superior mesenteric vein and inferior mesenteric vein along the mesocolon. Proximally, the venous return converges toward the portal venous system, with the superior mesenteric vein passing superiorly and posterior to the pancreas to meet the splenic vein at the portal confluence. Pelvic landmarks and lower lumbar vertebrae provide orientation. Clear midline relationships. Venous anatomy of the colon matters because it maps directly onto patterns of hematogenous spread and the physiology of portal hypertension. Colorectal carcinoma commonly seeds the liver via mesenteric venous drainage, and the segmental territories of the ileocolic, right colic, middle colic, left colic, and sigmoid veins parallel surgical resection planes and lymphovascular pedicles used in oncologic colectomy. This anterior view also helps clarify why inferior mesenteric venous flow can be routed into the splenic vein, a point that becomes practical when interpreting contrast CT or planning venous ligation while preserving adequate mesenteric outflow.
