Description

Arising from the ileocolic arterial system, small cecal arteries fan out over the anterior surface of the cecum, tracking along the taeniae coli and between haustral sacculations in the right lower quadrant. The ileum approaches the cecum superiorly and medially at the ileocecal junction, while the vermiform appendix descends from the inferomedial cecal pole. Pericolic fat and the adjacent mesenteric reflection frame the vascular arcade, and accompanying lymphatic vessels with clustered pericolic nodes are represented along the same routes. An anterior cecal vascular map matters when you are thinking about appendectomy and right hemicolectomy planning, because the anterior and posterior cecal branches can be sacrificed or preserved depending on how close the dissection runs to the ileocolic pedicle and the appendiceal artery. Ischemia at the cecal pole is a real complication, and it becomes more likely when the marginal supply is limited or when inflammatory adhesions from appendicitis distort the usual planes. Lymphatic drainage to pericolic and intermediate nodes along the ileocolic vessels also underpins oncologic resections for cecal carcinoma, where node harvest tracks the arterial anatomy.
