Description

Prominent in the abdominal and pelvic cavity are the ascending, transverse, descending, and sigmoid colon arranged in their usual frame around the small-intestinal loops, with the cecal region inferior and right-sided and the sigmoid colon descending into the left pelvis. Arterial branches course within the mesocolon to each segment, forming the marginal artery of Drummond as a continuous arcade along the mesenteric border from the right colic region across the transverse colon to the left colic and sigmoid territory. Proximally, ileocolic, right colic, and middle colic branches align with the superior mesenteric artery distribution, while left colic and sigmoid branches reflect the inferior mesenteric artery, meeting near the splenic flexure. Bony landmarks, including lower ribs, lumbar vertebrae, and the pelvic ring, anchor depth and orientation. Arterial anatomy of the colon matters because perfusion is segmental and variable, and the watershed zone around Griffiths point at the splenic flexure is a recurrent site of ischemic colitis when systemic hypotension or atherosclerotic disease limits collateral flow.
