Description
Spanning the abdomen in anterior view, the ascending colon lies on the viewer’s left and continues superiorly to the hepatic flexure, where it turns medially into the transverse colon before descending on the viewer’s right toward the splenic flexure. Arterial branches track along the mesenteric border, forming a continuous marginal artery (arterial arcade) that parallels the haustrated bowel wall from the cecal region across to the sigmoid colon and proximal rectum. Segmental vasa recta extend perpendicularly from the arcade to the colonic wall. Surface landmarks include the taeniae coli and appendices epiploicae, with small deposits of omental fat draped anteriorly. An anterior rendering of the colonic arterial pattern matters because it clarifies the territories of the superior mesenteric artery and inferior mesenteric artery and their anastomosis near the splenic flexure. That watershed zone, classically at Griffiths’ point, is a common site of ischemic colitis during low-flow states and after aortic or mesenteric interventions. Surgeons planning a right or left hemicolectomy also track the relationship of the marginal artery and vasa recta to determine ligation level while preserving perfusion to the remaining bowel.
