Description
Anteriorly, the sigmoid colon curves inferomedially into the rectum, with haustra fading as the bowel transitions into the straighter pelvic segment. Along the posterior abdominal wall, the inferior mesenteric artery continues as the superior rectal artery, descending in the mesorectum and dividing into paired terminal branches that run on either side of the rectum. More laterally within the pelvis, the middle rectal arteries approach the rectal wall from the internal iliac system, while the inferior rectal arteries course medially from the internal pudendal arteries toward the anal canal and perianal tissues. Small pararectal and sigmoid lymph nodes track alongside these vessels. A dense anastomotic field. Arterial supply to the rectum matters because it straddles classic watershed territory between hindgut and pelvic circulation, and that boundary shows up in the OR and in pathology. Low anterior resection and total mesorectal excision hinge on identifying the superior rectal pedicle and preserving autonomic nerves while controlling bleeding from middle rectal contributions, which can be variable and easy to underestimate from an anterior approach.
