Description
Dominating the upper abdomen, the liver sits superior to the stomach and transverse colon, with the gallbladder tucked on its inferior visceral surface near the right lobe. Blue hepatic veins course superiorly and posteriorly through the hepatic parenchyma to converge on the inferior vena cava at the posterior aspect of the liver, while red arterial branches and the larger portal venous channels ramify more inferiorly toward the porta hepatis. Portions of the stomach, pancreas, spleen, and loops of small and large intestine frame the hepatic outflow tract in anterior projection. Clear color separation supports rapid vessel identification. Anterior orientation helps clarify the relationship between hepatic venous drainage and adjacent portal inflow pathways, a point that often confuses learners when only the portal triad is emphasized. Hepatic vein anatomy has direct procedural relevance in transjugular intrahepatic portosystemic shunt (TIPS) planning, where the right hepatic vein is commonly selected for access into the portal venous system, and in hepatic resection where venous outflow preservation prevents postoperative congestion. Short hepatic veins entering the retrohepatic vena cava are a known source of brisk bleeding during mobilization of the right lobe. A surgical hazard.
