Description
Running along the medial aspect of the knee, the tibial (medial) collateral ligament appears as a broad, flattened band spanning from the medial femoral epicondyle and adjacent medial condyle to the medial surface of the proximal tibia. Anteriorly, the patella sits centered over the distal femur, with the patellar ligament descending inferiorly toward the tibial tuberosity. Articular cartilage caps the femoral condyles and tibial plateau, while the medial and lateral menisci interpose between them; the lateral collateral ligament is positioned more laterally and cord-like by comparison. Clinically, an anterior perspective on the medial collateral ligament helps clarify why valgus stress opens the medial joint line and why MCL injury commonly accompanies rotational trauma. Its intimate relationship with the medial meniscus matters. Deep fibers blend with the joint capsule and attach to the medial meniscus, so grade II to III MCL sprains can coincide with meniscocapsular separation and persistent medial joint line pain even when gross instability feels modest on exam.
