Description
Anteriorly oriented anatomy of the male knee places the patella and trochlear groove in the foreground, with the distal femur superior and the proximal tibia inferior. Along the medial side, the medial collateral ligament (tibial collateral ligament, ligamentum collaterale tibiale) spans from the medial femoral epicondyle to the anteromedial tibia, its superficial fibers blending with the medial joint capsule and lying superficial to the medial meniscus. Torn fibers are separated and frayed, with adjacent red shading marking the zone of disruption and tensile overload, while the quadriceps tendon and patellar tendon remain centered over the extensor mechanism anterior to the joint. Medial collateral ligament injury is the classic response to a valgus force at the knee, common in contact sports and falls with the foot planted. An anterior view makes the medial joint line easy to teach and to correlate with physical examination maneuvers, since tenderness at the medial femoral epicondyle or along the tibial insertion and pain with valgus stress testing at 30 degrees of flexion track directly to the superficial MCL.
