Description
Positioned deep to the infrahyoid plane, the paired sternothyroid muscles run superiorly from the posterior surface of the manubrium and adjacent first costal cartilage toward the oblique line of the thyroid cartilage. Their superior portions are partially obscured by the overlying sternohyoid, which lies more superficial and medial as it ascends to the hyoid bone. Lateral to the strap muscles, the sternocleidomastoid forms the prominent anterior neck contour, with the laryngeal prominence and upper tracheal region implied beneath the midline soft tissues. Fibers are clearly oriented longitudinally. A clean anterior relationship map. Sternothyroid matters because it is the infrahyoid muscle most directly tethered to the laryngeal framework, depressing the thyroid cartilage and larynx after elevation during swallowing and contributing to pitch control during phonation. In thyroidectomy and anterior cervical exposure, surgeons encounter sternohyoid first, then sternothyroid, and routine strap-muscle separation or division can change postoperative swallowing comfort and voice fatigue, even when the recurrent laryngeal nerve and external branch of the superior laryngeal nerve are preserved. This view also helps clarify why bulky goiter or postoperative hematoma can create anterior compartment tension that limits laryngeal excursion.
