Description
Presented on a transparent male thorax with the rib cage and thoracic spine visible, the esophagus descends in the posterior mediastinum, closely applied to the anterior surfaces of the vertebral bodies before passing through the esophageal hiatus of the diaphragm. Inferior to that junction, the stomach (ventriculus) lies predominantly in the left upper quadrant, with the fundus expanding superiorly beneath the left hemidiaphragm and deep to the lower ribs. The cardia sits just left of the midline, the body sweeps inferolaterally, and the greater curvature arcs laterally while the lesser curvature remains medial toward the liver. Orientation is clear. Linking the gastric fundus to the diaphragm and the lower thoracic skeleton matters when you teach or plan around gastroesophageal reflux disease, hiatal hernia, and antireflux surgery. Sliding hiatal hernia shifts the gastroesophageal junction superiorly into the thorax, and this relationship to the vertebral column and diaphragmatic hiatus is exactly what learners struggle to visualize in endoscopy reports and barium swallow correlation. The same anatomy guides safe dissection planes during laparoscopic fundoplication and helps explain why fundal gas can project beneath the left hemidiaphragm on plain radiographs.
