Description
Shown from the anterior aspect in anatomical position, the distal esophagus descends through the diaphragm to meet the stomach at the cardia in the epigastrium and left upper quadrant. The fundus domes superiorly and to the left beneath the left hemidiaphragm, while the body continues inferiorly toward the antrum and pylorus, which lie more medial and rightward as the organ turns into the proximal duodenum. The lesser curvature forms the concave right border and the greater curvature sweeps inferolaterally to the left, producing the classic J shaped gaster. Orientation is clear. This anterior perspective helps you teach the surface anatomy clinicians use when correlating symptoms to regions of the stomach: fundal gas under the left dome, antral pathology nearer the midline, and pyloric disease drifting toward the right upper abdomen. The relationship between the gastroesophageal junction and the diaphragmatic hiatus also supports discussion of sliding hiatal hernia and gastroesophageal reflux disease, where proximal migration of the cardia changes the pressure barrier at the lower esophageal sphincter. In surgery, the curvatures define the working lines for ligation of the gastroepiploic vessels along the greater curvature and the left and right gastric arteries along the lesser curvature during gastrectomy.
