Description
Arising from the anterior aspect of the axillary artery, the thoracoacromial trunk pierces the clavipectoral fascia and gives rise to the acromial branch, which courses superolaterally toward the acromion of the scapula. From an anterior perspective over the right shoulder, the vessel tracks deep to pectoralis major and toward the deltopectoral interval, where it approaches the anterior margin of deltoid and the coracoacromial region. The clavicle, proximal humerus, scapula, and upper ribs frame the route, while adjacent blue venous channels and light blue elements of the brachial plexus indicate the crowded neurovascular compartment around the axilla and shoulder girdle. Relationships matter here. For surgeons working through the deltopectoral approach, the thoracoacromial vessels are a common source of bleeding when elevating the cephalic vein and separating deltoid from pectoralis major, and the acromial branch contributes to the periarticular anastomoses around the acromion and acromioclavicular joint. This view also clarifies why trauma, iatrogenic injury, or postoperative scarring near the coracoid process and clavipectoral fascia can threaten regional perfusion and complicate flap planning. In shoulder arthroscopy and open reconstruction, the artery’s position relative to the clavicle and coracoacromial ligament serves as a dependable landmark.
