Description
Arising from the thyrocervical trunk of the subclavian artery, the inferior thyroid artery ascends in the lower cervical region before turning medially toward the posteroinferior aspect of the thyroid gland. From an anterior perspective, it sits deep to the sternohyoid and sternothyroid strap muscles and courses medial to the carotid sheath, with the common carotid artery and internal jugular vein positioned more laterally on each side. Superiorly, the mandible with dentition frames the lower face, and the hyoid bone lies midline above the laryngeal skeleton. Small thyroidal branches fan out toward the lower pole of the gland. Surgeons care about this vessel because it runs in the same corridor as the recurrent laryngeal nerve near the tracheoesophageal groove, and their relationship can vary from side to side, which is exactly where voice-threatening injury occurs during thyroidectomy. Ligating the inferior thyroid artery too far laterally risks devascularizing the parathyroid glands supplied by its branches, a common setup for postoperative hypocalcemia. Orientation matters. An anterior view clarifies how quickly the artery disappears deep to the infrahyoid muscles and why superficial landmarks can mislead in the operative field.
