Inferior Mesenteric Artery

Description

Centered in the anterior abdomen, the abdominal aorta descends along the vertebral column and gives rise to the inferior mesenteric artery (IMA) from its anterolateral aspect, typically at the level of L3. The IMA courses inferiorly and leftward, remaining anterior to the vertebral bodies and medial to the left psoas major, before continuing toward the pelvic brim where it transitions into the superior rectal artery. Adjacent venous anatomy includes the inferior vena cava on the right of the aorta, with renal arteries and veins branching laterally toward the kidneys. Bony landmarks from the rib cage to the pelvis bracket the field. Appreciating the IMA’s origin and trajectory matters in left-sided colorectal surgery, where high ligation versus preservation of the left colic artery can influence perfusion of the descending colon and colorectal anastomosis.
