Description
Arising near the abdominal aorta at the level of the celiac trunk, the inferior phrenic artery curves superiorly on the abdominal surface of the diaphragm, running deep to the peritoneum and anterior to the crura. From an anterior perspective in a male, its course is appreciated as it approaches the central tendon, with medial branches trending toward the esophageal hiatus and lateral branches sweeping toward the costal margin. Small ascending twigs may be seen anastomosing with the superior phrenic and lower intercostal arteries along the peripheral diaphragm. Orientation is everything. Because the inferior phrenic arteries often give rise to the superior suprarenal arteries, this relationship matters in adrenal pathology and in operative planning for suprarenalectomy, where inadvertent avulsion can lead to brisk retroperitoneal bleeding. Interventional radiology also relies on this artery as a frequent extrahepatic collateral supply to hepatocellular carcinoma, commonly via the right inferior phrenic artery, and it is a standard target for transarterial embolization when tumors abut the bare area of the liver and diaphragm. The anterior view clarifies why diaphragmatic irritation and referred shoulder pain can accompany subphrenic processes.
