Description
Arising as a terminal branch of the internal thoracic artery, the musculophrenic artery curves inferolaterally along the costal margin, running deep to the costal cartilages and adjacent to the anterior thoracic wall. Its course parallels the inferior border of the rib cage as it gives off anterior intercostal branches to the lower intercostal spaces, while continuing toward the peripheral muscular slips of the diaphragm. Superior to it, the internal thoracic artery descends parasternal before dividing into the musculophrenic and superior epigastric arteries. The diaphragm sits inferior and posterior as the target territory. For teaching and preoperative planning, this anterior perspective clarifies why the musculophrenic artery matters during chest tube placement and thoracoabdominal incisions near the costal margin. Injury to the internal thoracic system, including the musculophrenic branch, can contribute to brisk bleeding in anterolateral thoracostomy or during rib and costal cartilage exposure, and its anterior intercostal branches participate in the intercostal arterial anastomoses relevant to collateral flow when proximal vessels are compromised.
