Description

Arising from the anterior aspect of the abdominal aorta at the L3 level, the inferior mesenteric artery descends slightly to the left of midline before sweeping inferolaterally toward the left iliac fossa. From its proximal segment, the left colic artery courses superiorly and laterally toward the descending colon, while sigmoid arteries travel inferiorly within the sigmoid mesocolon. Distally, the main trunk continues as the superior rectal artery, crossing the pelvic brim to run along the posterior pelvic wall toward the rectum. The surrounding field logically includes the descending and sigmoid colon with haustra, adjacent small-bowel loops, and the inferior vena cava and tributaries in blue for venous context. Attention to this arterial pattern matters because the inferior mesenteric artery defines the hindgut supply and the watershed zones that influence colonic ischemia, most famously near the splenic flexure (Griffiths’ point) and rectosigmoid junction (Sudeck’s point). In left colectomy, sigmoidectomy, and low anterior resection, surgeons decide between high and low ligation of the inferior mesenteric artery based on oncologic clearance and preservation of perfusion through the marginal artery of Drummond.
