Description
Anterior neck anatomy is rendered with the thyroid gland seated inferior to the laryngeal framework and superior to the manubrium, with the middle thyroid vein emerging from the lateral thyroid parenchyma and coursing laterally to join the internal jugular vein within the carotid sheath. Medial to this venous channel lie the trachea and larynx, while the common carotid artery ascends just medial to the internal jugular vein, keeping the artery and vein in close parallel alignment. Superiorly, the mandible and dentition provide bony context, and inferior cervical vertebrae anchor the posterior midline. Short. Direct. That short trajectory is exactly why the middle thyroid vein matters in open thyroidectomy and neck dissection, where early identification and secure ligation can reduce venous oozing that obscures the recurrent laryngeal nerve and parathyroid planes. Its variability also explains occasional surprises: the vein may be duplicated, absent, or drain toward the brachiocephalic vein, and it can tether the thyroid lobe laterally during medial rotation. Close proximity to the carotid sheath places it at the crossroads of endocrine surgery and vascular safety. Use this plate to teach thyroid venous drainage in head and neck anatomy, to illustrate the surgical field for hemithyroidectomy or total thyroidectomy chapters, or to orient trainees to anterior cervical landmarks before central line placement near the internal jugular vein.
