Description
Centered in the anterior neck, the right and left lobes of the thyroid gland wrap anterolaterally around the upper trachea, joined across the midline by the isthmus at the level of the upper tracheal rings. Superiorly, the laryngeal framework is visible, including the thyroid cartilage and cricoid cartilage, with the trachea descending inferiorly in the midline. Red arterial channels track toward the gland from superolateral and inferolateral directions, while blue venous tributaries form a plexiform drainage pattern over the anterior surface before coursing laterally toward the internal jugular veins. Slender neural structures, consistent with the recurrent laryngeal nerves, ascend in the tracheoesophageal groove, immediately medial to the carotid sheath contents. Anterior vascular mapping of the thyroid and trachea matters in thyroidectomy and tracheostomy planning, where small-caliber but briskly bleeding branches can obscure the operative field in seconds. The superior thyroid artery and vein sit close to the external branch of the superior laryngeal nerve near the superior pole, while the inferior thyroid vessels relate to the recurrent laryngeal nerve near the cricothyroid junction, a common site of traction or ligature injury.
