Description
Seen from posterior in the male neck, the paired thyroid lobes wrap anterolaterally around the upper trachea while the isthmus crosses the anterior midline at the level of the upper tracheal rings; from behind, the posterior borders of the lobes frame the trachea and cricoid cartilage. Superior and inferior thyroid arteries approach the gland along its superolateral and inferolateral aspects, while a venous plexus drains as superior, middle, and inferior thyroid veins that course laterally toward the internal jugular and inferiorly toward the brachiocephalic veins. Fine arterioles and capillary networks drape across the fibrous capsule and track between lobules, closely applied to the tracheal adventitia. Posterior vascular anatomy matters because it is where thyroidectomy becomes unforgiving. The inferior thyroid artery and its branches run near the expected course of the recurrent laryngeal nerve in the tracheoesophageal groove, and the parathyroid glands usually sit on the posterior thyroid surface with a blood supply commonly derived from the inferior thyroid artery. Lose that supply and postoperative hypocalcemia becomes a real risk, even when the gland itself is removed uneventfully.
