Description
Arcing across the anterior neck, the ansa cervicalis forms a distinct nerve loop on the carotid sheath, with its superior root (descendens hypoglossi) traveling inferiorly after hitchhiking with the hypoglossal nerve (CN XII) and its inferior root (descendens cervicalis) arising from the ventral rami of C2 to C3. The loop lies deep to the sternocleidomastoid yet superficial to the internal jugular vein and common carotid artery, typically coursing along the anterolateral surface of the sheath. From this loop, slender motor branches run medially to the infrahyoid strap muscles, including the sternohyoid, sternothyroid, and the superior belly of omohyoid, which sit anterior to the laryngeal cartilages and upper trachea. Key neighboring landmarks, including the clavicles inferiorly and the scalene region laterally, frame the cervical plexus and proximal brachial plexus elements. Surgeons care about this loop because it is the workhorse donor for laryngeal reinnervation, most commonly ansa cervicalis to recurrent laryngeal nerve anastomosis in unilateral vocal fold paralysis, where its motor axons help restore tone without sacrificing major limb function. The same anatomy governs safety during carotid endarterectomy and anterior approaches to the carotid sheath, where traction or ligation can denervate strap muscles and complicate swallowing and phonation.
