Description
Seen from a lateral perspective in an adult male neck, the ansa cervicalis forms a distinct nerve loop on the anterior wall of the carotid sheath. The superior root (descendens hypoglossi, carrying C1 fibers) descends from the hypoglossal nerve (CN XII) just inferior to the angle of the mandible, while the inferior root (descendens cervicalis, typically C2 to C3) ascends from the cervical plexus to meet it. Superficial bony landmarks such as the mandible, skull base, and cervical vertebrae provide orientation, with the loop lying anterior to the prevertebral muscles and deep to the infrahyoid strap muscle plane. Cranial nerves and segmental spinal nerves are also rendered in continuity to anchor the ansa within the broader peripheral nervous system. Clinically, this is the nerve loop surgeons look for when operating in the carotid triangle or creating a window for tracheostomy and thyroid surgery. Injury or traction to the ansa cervicalis can weaken infrahyoid muscles, affecting laryngeal depression and swallowing mechanics, while its close relationship to CN XII makes it a practical landmark in differentiating hypoglossal palsy from isolated cervical plexus injury. This view also clarifies a common teaching point: the superior root travels with CN XII without being a true hypoglossal branch. Small structure, big consequences.
