Description
Seen from a lateral neck perspective, the superior root of the ansa cervicalis (descendens hypoglossi) emerges as C1 fibers that travel briefly with the hypoglossal nerve (CN XII) before descending on the anterior wall of the carotid sheath. Its course runs inferiorly and slightly anterior, typically superficial to the internal jugular vein and close to the common carotid artery, then arcs toward the inferior root (descendens cervicalis) from C2 to C3 to form the ansa loop. Surrounding landmarks in this corridor commonly include the sternocleidomastoid laterally, the strap muscles (sternohyoid and sternothyroid) more anterior and medial, and the clavicle and upper ribs at the inferior edge of the field. This lateral view matters because it clarifies where the ansa cervicalis sits relative to the carotid sheath, a relationship that drives both surgical planning and iatrogenic risk. During carotid endarterectomy, neck dissections, and submandibular approaches, traction or division of the superior root can denervate the infrahyoid muscles, leading to impaired laryngeal elevation and dysphagia, and the proximity to the internal jugular vein makes the nerve easy to mistake for a small venous tributary. The C1 hitchhiking segment along CN XII is the teaching point. It explains why lesions near the hypoglossal nerve can produce strap muscle weakness without a primary cervical plexus injury.
