Description
Running within the carotid triangle, the ansa cervicalis forms a loop superficial to the carotid sheath, with its superior root (descendens hypoglossi, C1 fibers traveling with the hypoglossal nerve) descending from the region just inferior to the posterior belly of digastric and its inferior root (descendens cervicalis, typically C2 to C3) ascending from the lateral neck. The completed loop lies anterior and medial to the internal jugular vein and common carotid artery, then distributes branches on the anterior surface of the infrahyoid muscles. Sternocleidomastoid sits laterally as a dominant landmark, while the laryngeal framework and strap muscles define the midline compartment. Tight anatomy. Surgeons care about this configuration during carotid endarterectomy and neck dissection, where retraction around the carotid sheath can stretch or transect the loop, producing denervation of sternohyoid, sternothyroid, and omohyoid with postoperative dysphonia or impaired depression of the hyoid and larynx. The illustration also supports teaching the frequent anatomic variants, including a high or low loop, contribution from C4, or a hypoglossal-descending branch that mimics the ansa in the operative field. Relationship to the internal jugular vein is the key spatial cue.
