Description
Anterior head and neck anatomy is presented with the dermal layer rendered translucent to reveal the buccinator as a flat muscular sheet forming the lateral wall of the oral cavity. The buccinator spans between the maxilla and mandible in the molar region, lying deep to the superficial cheek soft tissues and anterior to the masseter along the ramus of the mandible. Superiorly it relates to the maxillary alveolar process and upper molars, while inferiorly it approaches the mandibular alveolar process and the level of the oral vestibule. Teeth, facial bones, and adjacent structures of the nasal and orbital regions provide bony context for the cheek. Buccinator anatomy matters because it is the working interface between mastication and oral continence, compressing the cheek against the molars to keep the bolus on the occlusal surfaces while coordinating with orbicularis oris at the modiolus. Surgeons and dentists track the parotid (Stensen) duct as it courses across the masseter and turns medially to pierce the buccinator, opening into the oral vestibule opposite the maxillary second molar, a frequent landmark during intraoral procedures and a risk point in cheek lacerations. This is a common site of injury. The same corridor is relevant in buccal space infections of odontogenic origin and in evaluating buccal branch facial nerve deficits that alter cheek tone and contribute to food trapping.
