Description
Anteriorly, the buccinator is presented as a flat, quadrangular sheet forming the deep muscular wall of the cheek, lying lateral to the oral cavity and converging toward the modiolus at the angle of the mouth (oral commissure). Superficial facial muscles frame it, with zygomaticus major descending inferomedially toward the commissure, orbicularis oris encircling the oral fissure, and orbicularis oculi closing the palpebral fissure superiorly. Buccal soft tissues overlay the buccinator laterally, while the lips and nasolabial region anchor the anterior facial landmarks. Clean midline symmetry supports side-to-side comparison. Functionally, the buccinator compresses the cheek against the teeth to keep the bolus on the occlusal surfaces during mastication and to control intraoral pressure for blowing and speech. That makes its relationships at the modiolus and along the vestibule clinically concrete, not academic. The parotid (Stensen) duct pierces the buccinator to open opposite the maxillary second molar, a key point for sialendoscopy, duct cannulation, and for understanding why duct injury can complicate cheek lacerations or transbuccal approaches.
