Centered in the upper abdomen, the lobulated pancreas lies transversely with its head nestled within the C-shaped duodenum, while the body extends leftward toward the splenic hilum and the tail approaches the spleen. The duodenum curves from the superior part into the descending segment along the right side of the pancreatic head, then sweeps inferiorly and medially before continuing as jejunum. Along the superior border of the pancreatic body, the splenic artery courses tortuously toward the spleen, with the splenic vein running straighter and posterior to it, converging toward the portal venous system. Endocrine elements are implied by the parenchymal texture representing pancreatic acini and islets of Langerhans. An anterior perspective like this clarifies why pathology in one structure often presents as disease in the other. In acute pancreatitis, inflammation and edema at the pancreatic head can narrow the adjacent second part of the duodenum and complicate gastric outlet symptoms, while chronic pancreatitis may scar the head region and distort the periduodenal contour seen on endoscopy or cross-sectional imaging. Surgeons also rely on this relationship during pancreaticoduodenectomy (Whipple procedure), where safe dissection depends on recognizing the pancreatic head’s intimate wrap by the duodenum and the proximal course of the splenic vessels when mobilizing the gland.
