Description

Centered in the upper abdomen, the duodenum curves in a C shape around the pancreatic head, with the pancreatic body and tail extending leftward toward the splenic hilum. From an anterior perspective, green lymphatic vessels track along the pancreaticoduodenal arcades and the course of the splenic artery, converging on pancreaticoduodenal, pancreaticosplenic, and splenic hilar lymph nodes. Superiormedially, lymphatics also follow the common bile duct toward hepatic nodes, while inferiorly the mesenteric root provides continuity with intestinal (superior mesenteric) nodal basins. Short efferent channels leave the spleen at the hilum and run medially along the tail of the pancreas. This is the drainage map that explains early spread patterns in periampullary and pancreatic malignancy, where nodal disease commonly appears along the superior mesenteric vessels and at the celiac axis rather than remaining confined to the gland. It also matters in distal pancreatectomy with splenectomy, where lymphadenectomy frequently includes nodes along the splenic artery and splenic hilum, and in chronic pancreatitis where lymph node enlargement can mimic metastatic disease on cross sectional imaging. Small nodes, big consequences. Educators can place this plate beside CT or MRI anatomy to teach celiac, superior mesenteric, and splenic nodal stations in GI surgery and oncology modules, and authors can pair it with text on pancreatic cancer staging, Whipple procedure margins, or splenic immune function in hematology and immunology chapters. It also fits well in patient facing materials that need a clear explanation of why lymph nodes near the pancreas and spleen are sampled.
