Description
Oriented from a posterior perspective, the duodenum curves in a C loop around the head of the pancreas, while the pancreatic body extends leftward across the posterior abdominal wall toward the splenic hilum. Along the superior border of the gland, the splenic artery typically tracks tortuously toward the spleen, while the splenic vein courses more posteriorly and inferiorly before joining the superior mesenteric vein to form the portal vein. The spleen sits in the left upper quadrant, lateral to the pancreatic tail, tethered to the posterior body wall by the lienorenal (splenorenal) ligament and separated from the left kidney by that peritoneal fold. Key back wall relationships read clearly. A posterior angle matters because it maps the retroperitoneal fixation of the duodenum (second through fourth parts) and the pancreas, the exact relationship that governs how these organs are mobilized in surgery. During a Kocher maneuver, the duodenum and pancreatic head are reflected medially to expose the inferior vena cava and aorta, and this view helps anticipate where the bile duct and gastroduodenal artery sit relative to the pancreatic head. It also frames the pancreatic tail as it approaches the splenic hilum, a frequent site of iatrogenic injury to the splenic vessels during distal pancreatectomy and splenectomy. Short.
