Description
Running along the anterior forearm, the flexor carpi radialis (FCR) muscle belly lies superficial in the central to radial portion of the flexor compartment, tapering distally into a cord-like tendon that becomes prominent as it approaches the wrist. Medially, the FCR typically parallels the palmaris longus tendon when present, while laterally it borders the brachioradialis and overlies the radius. Deep to the muscle belly you would expect the flexor digitorum superficialis and pronator teres proximally, with the radius and ulna forming the osseous scaffold and the carpal bones and metacarpals continuing distally into the hand. Skin is retained as a semi-transparent envelope to keep surface relationships readable. Clinically, the FCR tendon is a reliable surface landmark on the volar wrist: the radial artery runs just lateral to it, and the median nerve courses deeper and slightly medial before entering the carpal tunnel. That anatomy matters in radial artery cannulation, volar ganglion assessment, and when planning the classic volar (Henry) approach for distal radius fixation, where the interval between FCR and the radial artery is developed to protect vascular structures. Tendinopathy at the FCR sheath near the trapezium can also mimic first carpometacarpal pain. A small tendon with outsized teaching value.
