Description
Running along the ulnar border of the anterior forearm, the flexor carpi ulnaris (FCU) forms a superficial muscular column from the medial epicondyle of the humerus and proximal ulna down toward the wrist. Its muscle belly lies medial to flexor digitorum superficialis, with the ulna immediately deep and the ulnar neurovascular bundle coursing along its radial (lateral) margin in the distal forearm. Distally, the FCU narrows into a tendon that crosses the anterior aspect of the wrist to insert on the pisiform and, through the pisohamate and pisometacarpal ligaments, continues toward the hook of hamate and base of the fifth metacarpal. Medial. Unmistakable. An anterior perspective matters because FCU is the dominant surface landmark for the ulnar side of the flexor compartment and for orienting the examiner to the ulnar nerve and ulnar artery as they pass into Guyon’s canal at the wrist. Proximally, the humeral head contributes to the common flexor tendon origin, a frequent pain generator in medial epicondylitis, while the ulnar head and adjacent cubital tunnel region frame clinical patterns of ulnar neuropathy that can be provoked by repetitive elbow flexion or sustained pressure. Surgical approaches to the ulnar wrist and hypothenar region often reference the pisiform and FCU tendon to avoid iatrogenic injury to the ulnar nerve branches. Use this asset in upper limb gross anatomy courses to teach compartmental organization and tendon insertions, or in hand surgery and sports medicine materials discussing FCU tendinopathy, medial epicondylitis, and ulnar-sided wrist pain. It also fits rehabilitation guides that cue palpation of the FCU tendon at the pisiform during resisted wrist flexion with ulnar deviation.
