Description
Running along the ulnar border of the posterior forearm, the flexor carpi ulnaris is presented beneath the skin with its muscular belly broad proximally and tapering distally into a discrete tendon approaching the ulnar side of the wrist. Proximally, the ulnar head arises adjacent to the olecranon process and posterior border of the ulna, while the humeral head relates to the medial epicondyle of the humerus, framing the medial aspect of the elbow joint. Lateral to it, the radius and a partial brachioradialis mass provide a contrasting landmark toward the radial side, and distally the carpal bones, metacarpals, and phalanges establish the hand orientation with the dorsum facing the viewer. Orientation matters here because the flexor carpi ulnaris is often taught from an anterior forearm dissection, yet the posterior relationship to the olecranon and subcutaneous ulnar border is a reliable surface landmark during examination and operative planning. The ulnar nerve passes deep to the flexor carpi ulnaris after traversing the cubital tunnel behind the medial epicondyle, so hypertrophy, scarring, or surgical dissection around the proximal muscle belly can directly affect symptoms of cubital tunnel syndrome. A critical landmark. Distally, the tendon’s course toward the pisiform (with expansions to the hook of hamate and base of the fifth metacarpal via the pisohamate and pisometacarpal ligaments) explains why ulnar-sided wrist pain and altered grip mechanics can track to this single musculotendinous unit. Hand and upper-limb anatomy courses will use this posterior perspective to reinforce medial versus lateral forearm orientation, olecranon palpation, and the functional line of pull for wrist flexion and ulnar deviation.
