Description

Arising from the lumbar plexus (predominantly L1), the ilioinguinal nerve is traced from the lateral border of psoas major across the anterior surface of quadratus lumborum, then coursing inferolaterally toward the iliac crest. It pierces transversus abdominis near the anterior part of the iliac crest and continues between transversus and internal oblique, running superior and parallel to the inguinal ligament as it approaches the inguinal canal. Distally, it passes through the superficial inguinal ring to reach the anterior scrotum and root of the penis, with cutaneous territory extending to the superomedial thigh. Landmarks in this anterior presentation typically include the lumbar vertebrae, iliac crest, anterior superior iliac spine, inguinal ligament, and pubic tubercle, providing clear medial to lateral orientation in the male pelvis. Clinical correlation sits right at the canal. The ilioinguinal nerve is a common source of chronic post-herniorrhaphy groin pain after open inguinal hernia repair, where it can be entrapped by sutures, mesh, or postoperative fibrosis near the superficial ring and along the inguinal ligament.
