Description
Arising from the posterior division of the internal iliac artery, the iliolumbar artery is traced in anterior view as it courses superiorly and laterally toward the medial iliac fossa, running deep to the psoas major and near the lumbosacral junction. Surrounding bony landmarks include the lumbar vertebrae with intervertebral discs, the sacrum, and the right and left hip bones (ilium, ischium, and pubis) forming the pelvic ring, with the femoral heads seated in the acetabula inferiorly. Proximal inflow from the abdominal aorta into the common iliac arteries is suggested, framing the iliolumbar branch in context with pelvic arterial anatomy. Because it climbs toward the iliac crest and gives lumbar and iliac branches, the iliolumbar artery matters when you are teaching collateral pathways between the internal iliac and lumbar arteries and when you are explaining why posterior pelvic bleeding can persist after proximal control. It also sits in a neighborhood relevant to anterior approaches to the lumbosacral spine and pelvic brim exposure, where injury can occur during retroperitoneal dissection along the medial iliac fossa or psoas.
