Description
Intercondylar eminence of the proximal tibia dominates this anterior, superior-facing look at the tibial plateau, rising between the medial and lateral articular surfaces that accept the corresponding femoral condyles. Medial and lateral tibial spines (intercondylar tubercles) form the peak of the eminence, with the anterior intercondylar area spreading inferiorly and anteriorly toward the anterior margin of the plateau. The irregular intercondylar region sits medial to lateral between the smooth hyaline cartilage bearing surfaces, while the bony margins of the condyles frame it on either side. Attachment anatomy in the intercondylar area is where knee stability becomes concrete: the anterior cruciate ligament inserts on the anterior intercondylar area just anterior to the eminence, while the posterior cruciate ligament anchors to the posterior intercondylar area, and the anterior horns of the medial and lateral menisci attach near the anterior intercondylar region adjacent to the tibial spines. This is a high-yield arthroscopic map. Orthopedic readers will recognize the tibial spine as the site of ACL avulsion injuries (tibial eminence fractures) in pediatric patients, and the intercondylar topography as a reference when discussing meniscal root tears and tibial tunnel placement for ACL reconstruction. The fixed anterior viewpoint makes the medial versus lateral relationships of the spines and adjacent plateau margins easy to communicate without conflating them with the tibial tuberosity or diaphyseal landmarks.
