Description
Arising from the parotid and masseteric fascia over the lateral cheek, the risorius courses anteromedially as a thin, superficial slip toward the angle of the mouth. Its fibers insert into the modiolus, blending with orbicularis oris and neighboring perioral elevators and depressors, while the zygomaticus major lies superior and runs more obliquely from the zygomatic bone to the oral commissure. From an anterior perspective in a male face, the muscle sits lateral to the oral fissure and superficial to the buccinator, defining a subtle line of tension across the cheek. Small but distinct. Functionally, risorius retracts the labial commissure laterally, contributing to the transverse smile and expressions such as grimacing, and it provides a useful teaching counterpoint to the stronger elevation produced by the zygomaticus major. Because its fibers interdigitate at the modiolus, this view helps clarify why perioral asymmetry can follow facial nerve injury, with buccal branch weakness reducing lateral commissure retraction on the affected side. Cosmetic and reconstructive planning also benefits from understanding this superficial anatomy, since botulinum toxin placement or dissection in the lateral perioral region can alter smile width and commissure position.
