Description

Arising from the popliteal artery, the superior lateral genicular artery courses laterally and anteriorly, skirting the superior border of the lateral femoral condyle beneath the distal fibers of vastus lateralis and along the lateral edge of the femur near the lateral epicondyle. From this anterior perspective, its relationship to the knee joint line becomes clear, with the vessel passing proximal to the lateral collateral ligament region and contributing to the periarticular genicular anastomosis around the patella and femoral trochlea. Adjacent arterial channels from the superior medial genicular, inferior lateral genicular, and anterior tibial recurrent territories may be suggested as they converge around the capsule. Small-caliber arterioles and accompanying veins track alongside the named artery. This artery matters when the knee’s primary inflow is compromised, because the genicular network can maintain distal perfusion in superficial femoral artery occlusion and after femoropopliteal bypass. The course above the lateral femoral condyle also places it at risk during lateral approaches to the distal femur and knee, including lateral parapatellar arthrotomy, distal femoral osteotomy, and hardware placement for lateral femoral condyle fractures. Bleeding from a genicular branch is rarely dramatic, but it is persistent and obscures the lateral gutter fast.
