Running along the anteromedial aspect of the knee, the superior medial genicular vein courses transversely across the distal femur and upper medial joint line, positioned superior to the medial tibial plateau and anterior to the posteromedial capsule. Proximally, its smaller venules converge from the periarticular plexus and track toward the femoral venous system, while distally it communicates with the venous channels around the medial head of gastrocnemius. Both lower limbs are presented from the anterior perspective, so the vein is read in relationship to the medial patellar border and adjacent superficial pathways such as the great saphenous vein along the medial leg. For clinicians, this vessel matters because the genicular venous network becomes conspicuous in postoperative knees and in inflammatory arthropathies, where periarticular hypervascularity and edema can distort normal planes.
