Description

Arising from the anterior aspect of the abdominal aorta just inferior to the celiac trunk, the superior mesenteric artery descends anterior to the left renal vein and crosses the third part of the duodenum before entering the root of the mesentery. From this anterior perspective, proximal jejunal and ileal branches radiate left and inferior within the small-bowel mesentery, while the middle colic, right colic, and ileocolic arteries course toward the transverse and ascending colon and cecum. Marginal arterial arcades track along the mesenteric border of the colon, with vasa recta running perpendicularly toward the haustrated wall. Epiploic appendages fringe the colonic surface. That steep aortomesenteric descent is more than a textbook line. Compression between the SMA and aorta explains superior mesenteric artery syndrome with third-part duodenal obstruction, and the same angle can entrap the left renal vein in nutcracker syndrome. Surgeons also use these branches as hard landmarks during right hemicolectomy and extended colectomy, since perfusion of the hepatic flexure depends on the middle colic and right colic territories and their variable anastomoses. Ischemic colitis respects these watershed boundaries. Medical educators can drop this plate straight into gross anatomy teaching on midgut derivatives, the mesentery, and colonic arterial arcades, or into radiology and surgery materials that correlate CTA findings with SMA branch anatomy.
