Description
Anteriorly, the paired temporalis muscles occupy the temporal fossae on either side of the calvaria, their fan-shaped fibers converging inferiorly into a thick tendon that passes deep to the zygomatic arch toward the coronoid process of the mandible. Superficial anterior facial musculature is seen in front of this field, including the frontalis over the forehead, orbicularis oculi around the palpebral fissures, and the zygomaticus group coursing inferomedially toward the oral commissure. Inferiorly, the masseter and buccinator region frames the mandibular ramus and cheek, while the platysma and sternocleidomastoid define the anterior neck contours. Fiber direction is the teaching point. Temporalis anatomy matters when you need to distinguish elevation from retrusion of the mandible, because the more anterior fibers run almost vertically while posterior fibers angle to pull the mandible superiorly and posteriorly during closing. Clinically, this relationship under the zygomatic arch explains why coronoidectomy for temporalis tendon release can restore mouth opening in coronoid hyperplasia, and why temporalis hypertrophy or myofascial trigger points often present as temporal headache that worsens with mastication. The anterior perspective also supports surgical planning for a frontotemporal (pterional) approach, where dissection and reflection of temporalis and its fascia helps protect the frontal branch of the facial nerve as it crosses the zygomatic arch in the temporal region.
